Not sure how to get that going on the home.php file…..I’m quite sure it’s possible, but it looks like it would take some knowledge of PHP to get this setup.
Have you heard or used Gravity Forms? It is a premium plugin that does cost money (around $30) but it is definitely worth the price. Everyone I know uses it since it’s the best one out there.
