I’d like to add this form script :

into the index.php file of the Epik childtheme. Is this possible?
The Epik theme doesn’t have an index.php file, it’s a child theme. If your tutorial requires an index.php file, you’ll have to make edits to the Genesis Framework (through hooks or filters in the functions file I assume)

Not sure how to get that going on the home.php file…..I’m quite sure it’s possible, but it looks like it would take some knowledge of PHP to get this setup.
Have you heard or used Gravity Forms? It is a premium plugin that does cost money (around $30) but it is definitely worth the price. Everyone I know uses it since it’s the best one out there. You can setup forms easily without needing to add any code. Check it out if you haven’t already – http://gravityforms.com
